비주얼 깡패 literally means "visual bully" but it refers to someone who is very good looking. There are many expressions that end with 깡패. 어깨 깡패 is someone whose shoulder are very wide, usually from working out and 실물 깡패 is someone or something that looks way better in person than in photos.
